Here you will find forms and necessary document collection. To receive your certification, these forms must be filled out and these documents must be sent in.

IMPORTANT: Make note of your Class. You will need this information to complete your Enrollment Documents.

BE ADVISED: Select your information carefully as your registration deposit is NON-TRANSFERABLE after 30 days and completely NON-REFUNDABLE. If you request a change of start date, you may have to re-register and pay another registration fee.

 

Any associated kit fees are non-refundable. Take-home kits are a gift upon completion of the class, not an obligatory portion of the class, as all school materials are supplied.
